GENERAL CONDITIONS:
  1. PRE-INSPECTION MEETING: THE PERMITTEE SHALL CONTACT THE BUREAU OF CONTRACT ADMINSTRATION (INSPECTION) AT (213) 485-5080 FOR A PRE-INSPECTION MEETING PRIOR TO THE START OF WORK. THE FINE FOR WORKING WITHOUT REQUESTING INSPECTION IS $4,000. (LAMC 62.61(e)(1)).
  2. UTILITIES HAVE ONLY BEEN CHECKED TO THE EXTENT OF 10-FT ON BOTH SIDES OF THE TRENCH IN THIS PLAN.
  3. PRIOR TO STARTING CONSTRUCTION, IT IS THE SOLE RESPONSIBILITY OF THE CONTRACTOR TO LOCATE ALL EXISTING UTILITIES AND CONDUITS, REGARDLESS OF WHETHER OR NOT THEY ARE SHOWN ON THE PLANS, AND TO TAKE APPROPRIATE MEASURES TO PROTECT THEM FROM ANY POTENTIAL DAMAGE.
  4. RECENTLY RESURFACED STREET: THE ISSUANCE OF THIS PERMIT DOES NOT GRANT THE PERMITTEE PERMISSION TO PROCEED TO CUT, EXCAVATE OR DAMAGE A STREET PAVEMENT WHEN THE STREET, THROUGH SITE OBSERVATION, APPEARS TO BE RECENTLY RESURFACED. IN SUCH A CASE, THE PERMITTEE SHALL NOT PROCEED WITH ANY OF THE PERMITTED WORK AND MUST IMMEDIATELY CONTACT THE BUREAU OF ENGINEERING DISTRICT OFFICE THAT ISSUE THE PERMIT. IF THE PERMITTEE PROCEEDS TO WORK WITHOUT CONTACTING THE BUREAU OF ENGINEERING DISTRICT OFFICE, ANY DAMAGE TO A STREET RESURFACED WITHIN ONE YEAR WILL REQUIRE THE PERMITTEE TO REPAVE THE ENTIRE STREET WIDTH FROM BLOCK TO BLOCK.

DECORATIVE IMPROVEMENTS: PERMITTEE SHALL STOP WORK AND CONTACT THE BUREAU OF ENGINEERING PRIOR TO CUTTING OR EXCAVATING ANY DECORATIVE SIDEWALK, PAVEMENT, OR CROSSWALK, UNLESS ALREADY ADDRESSED IN THE PERMIT OR PLANS. ANY UNAUTHORIZED DAMAGE TO THE DECORATIVE SIDEWALK, PAVEMENT, OR CROSSWALK MUST BE REPAIRED OR RECONSTRUCTED IN KIND BY THE PERMITTEE, AS DIRECTED BY THE BUREAU OF ENGINEERING, IN A MANNER SATISFACTORY TO THE INSPECTOR OF PUBLIC WORKS.

  21. THIS PERMIT GRANTS NO AUTHORITY FOR THE PERMANENT OCCUPATION OF THE PUBLIC RIGHT-OF-WAY BY ANY CONDUITS, PIPES, CABLES, WIRES, OR EQUIPMENT OF ANY SORT WHATSOEVER. SEE MUNICIPAL CODE SECTION 62.02(a). BY ISSUING THIS PERMIT THE CITY OF LOS ANGELES MAKES NO REPRESENTATION THAT ANY STATE STATUTE CREATES SUCH AUTHORITY.   2. CALL FOR INSPECTION OF PERMANENT RESURFACING NO LATER THAN NOON OF THE PRECEDING WORK DAY.
  3. ALL CHANGES IN SKETCHES AND PERMIT LIMITS MUST HAVE PRIOR APPROVAL BY THE CITY'S BUREAU OF ENGINEERING.
  4. PEDESTRIAN AND VEHICULAR ACCESS MUST BE MAINTAINED AT ALL TIMES.
  5. IF BARRICADES AND PROTECTIVE DEVICES ARE NOT FURNISHED BY THE CONTRACTOR AS REQUIRED, THE CITY OF LOS ANGELES MAY, AT ITS OPTION, PROVIDE THEM FOR A FEE TO THE CONTRACTOR.
  6. TRAFFIC REQUIREMENTS: ALL TRAFFIC LANES IN MAJOR, SECONDARY, AND COLLECTOR DESIGNATED STREETS SHALL BE UNOBSTRUCTED FROM 6 AM TO 9 AM AND 3:30 PM TO 7 PM. ADDITIONAL TRAFFIC LANES MAY BE CLOSED AT ALL OTHER TIMES WITH A LADOT APPROVED TRAFFIC CONTROL PLAN FOR THIS PERMIT. Contractor shall be responsible for the protection of public and private property adjacent to the work and shall exercise due caution to avoid damage to such property. Whenever a sidewalk or curb is removed and replaced to accommodate a utility installation, any and all survey monuments must be preserved or replaced by the utility or their contractor.

The contractor shall repair or replace “in-kind” all existing improvements within the right-of-way which are not designated for removal (e.g., curbs, sidewalks, driveways, fences, walls, signs, installations, pavements, structures, etc.) Which are damaged or removed as a result of its operations.

The contractor shall coordinate with the property owners for an access plan to the satisfaction of the inspector.

City street furniture: construction activity is within 5 feet of city transit furniture (e.g. transit shelter, public amenity kiosk, news stand vending kiosk, automated public toilet, bus bench, associated trash receptacle), the permittee/contractor shall arrange and pay for the removal, storage, relocation, replacement, and/or re-installation of all transit furniture and their related appurtenances, whether shown and indicated on the plans or not. Transit furniture are the property of the city of la and may only be removed by the city's authorized vendors.
