SPECIAL PERMIT REQUIREMENTS:
Trench backfill and a/c pavement resurfacing shall follow the latest version of la city std plan s-477-2.
One sack slurry within street. 1.5 sack slurry or one sack slurry with in-kind compacted base material in concrete streets/sidewalk. Native soil backfill in landscape and dirt.
Damaged concrete is to be restored in kind to the nearest built scoreline. Affected landscaping is to be restored in kind.
Contractor shall be responsible for the protection of public and private property adjacent to the work and shall exercise due caution to avoid damage to such property. Whenever a sidewalk or curb is removed and replaced to accommodate a utility installation, any and all survey monuments must be preserved or replaced by the utility or their contractor.
The contractor shall repair or replace “in-kind” all existing improvements within the right-of-way which are not designated for removal (e.g., curbs, sidewalks, driveways, fences, walls, signs, installations, pavements, structures, etc.) Which are damaged or removed as a result of its operations.
The contractor shall coordinate with the property owners for an access plan to the satisfaction of the inspector.
City street furniture: construction activity is within 5 feet of city transit furniture (e.g. transit shelter, public amenity kiosk, news stand vending kiosk, automated public toilet, bus bench, associated trash receptacle), the permittee/contractor shall arrange and pay for the removal, storage, relocation, replacement, and/or re-installation of all transit furniture and their related appurtenances, whether shown and indicated on the plans or not. Transit furniture are the property of the city of la and may only be removed by the city's authorized vendors. Advance arrangement shall be made 6 weeks in advance of required work.
